You have been asked to see Mr. Robert Johnson, a 76-year old man who has asked for an appointment because he had an episode of loss of consciousness the previous week.

His family insisted that he asked for an appointment as they felt that he had not been feeling 'quite right'.

Please take a focused history, address the patients concerns, offer your differentials, investigations and devise and management plan.
